5-step registration process

Step 1: Choose Your Program

Select the program you are registering for (Cheer or Football) to ensure your athlete is placed correctly.

Step 2: Complete the Athlete Information Form

Fill out all required athlete details, including contact information, age, grade, and emergency contacts.

Step 3: Review & Sign League Rules (Sport-Specific)

Please carefully read and electronically sign the league rules for your selected sport.

• Cheer: Review cheer-specific expectations, safety guidelines, and conduct standards.

• Football: Review football-specific rules, equipment requirements, and safety protocols.

Step 4: Complete the Athlete Social Media Form

Grant or decline permission for your athlete to appear in photos and videos used for league marketing and promotion.

Step 5: Submit the Registration Form Below

Once all forms are completed, submit your registration to officially secure your athlete’s spot.

The Phoenix Athletix League provides secondary insurance coverage for every participant. This coverage will apply only after the participant’s primary insurance has been used and does not cover the full claim. If the participant does not have primary insurance, the league’s secondary insurance will become primary.

Consent and Liability Waiver

I give my consent for the above-named participant to enroll in the Phoenix Athletix program. I acknowledge and assume all risks and hazards incidental to his/her participation in the program.

I hereby waive, release, indemnify, and hold harmless Phoenix Athletix, LLC, including its employees, volunteers, sponsors, coaches, and participants, from any and all claims, demands, or causes of action arising from injuries, damages, or losses sustained as a result of participation in the program.

Uniform & Equipment Agreement

By completing registration, you acknowledge and agree to the following:

Uniforms provided by Phoenix Athletix are for the athlete to keep at the end of the season
Equipment items such as pom poms, helmets, shoulder pads, and Padded pants must be returned to the league at the conclusion of the season or as requested
For football, athletes will keep their jersey only; all other league-issued equipment must be returned.
For cheer, athletes will keep their full cheer uniform; any additional equipment must be returned as directed.

I understand that all issued equipment must be returned promptly at the end of the season. Failure to return equipment will result in a $100 equipment fee.

Registration Refund Policy

Registration fees are fully refundable up until the official registration closing date July 1st. Refund request must be submitted in writing on or before 07/01 to be approved. Once registration closes on July 1st, all registration fees become non-refundable due to roster finalization, staffing, scheduling, and program cost commitments. By submitting your registration, you acknowledge and agree to this refund policy.
